STEP-BY-STEP INSTRUCTIONS FOR SUBMITTING DOCUMENTS
1. In the educational institution, write an application for enrolling the child in the 1st grade.
2. At the educational institution, fill out two consent forms for the use and processing of personal data (yours and your child's).
3. In the educational institution, sign the consent for the psychological support of the child while studying in the educational institution.
4. You must have with you a package of documents that are attached to the application:
- a copy of the child's birth certificate;
- a copy of any document certifying the child's place of residence at the address of the microdistrict in accordance with the territory served by our educational institution;
- map of preventive vaccinations (063/0);
- child's health certificate No. 086-1/0 (commission).
